Direct Booking Clients:
- Adult therapy form: Client Information Form
- Minor therapy form: Minor Intake Pack (to be signed by both biological parents)

Direct Clients Therapy Fee Structure
Direct Payment: R850 per 50min session
Medical Aid Claims: R990 per 50min session

Employee Wellness Clients:
- Careways form             : Careways Statement of Understanding
- Universal Health form : Universal Statement of Understanding
- Lifeassist form              : Lifeassist Agreement 
- Reality Wellness form  : Reality Wellness Consent Form
